Chiari malformation (CM) is one of the most common malformations central nervous system. It may be seen with syringomyelia, hydrocephalia and associated pathologies. We described an uncommon clinical presentation CM in this paper. report a case 45 years old male patient consulted to our outpatient clinic claw hand deformity who had misdiagnosed as ulnar entrapment neuropathy undergone transposition surgery. performed electroneuromyography (ENMG) revealed normal conduction nerve. focused on other causes found type-I syrinx cavity. neurosurgery center for further treatment. CMI often syringomyelia. Claw oftenly traumatic or tumor related cavity but association very unusual. recommend evaluation such patients more carefully focusig different causes.
